Harvests shall be reported for each deer taken in the Co-op. One user may submit multiple forms; one for each deer taken, regardless of hunter. Fill out each field per the guideline below. If not all of the information is available, leave those fields blank.
Date - Select the date from the pull-down calendar or manually type in the date yourself for the day the deer was taken.
Property - Select the property from the pull-down menu from which the deer was taken.
Hunter Name - Type the hunter's name who took the deer.
Type - Select the type of deer taken:
2.5+ year old buck
1.5 year old buck
Button Buck
Doe - Does that are 1.5 years or older
Doe Fawn - Females 0.5 year old
Lactating - If the deer is an adult doe, check this box if it is lactating.
Age - If jaw bone aging is available, provide estimated age.
Weight (lb.) - Field dressed weight.
Antler Points - Number of points.
Antler Width (in.) - If a buck, outside width of antlers.
Antler Circum (in.) - If a buck, circumference of antler at the base.
Upload Photo - If you have any photos of the deer, either on trail camera, or after harvest, or pictures of the jaw bone, please upload those here.
When completed, click the submit button to submit your report. Be sure to report your harvest to the NYS DEC! This form not a substitute for reporting game harvests to the DEC.
